Information we need from you
To help us answer your query, have the following information ready to tell us over the phone or by email.
For a general enquiry
Provide:
- your full name
- your email address
- your phone number
- the BDM product or service you’re enquiring about.
For a current or past order
Provide details of the order including:
- the type of product or service applied for
- the applicant’s:
- full name
- date of birth
- delivery address.
Example
A birth certificate for:
- John Michael Smith
- date of birth — 1 February 2012
- delivery address — 1A Queen Street, Auckland 1010.
Also provide:
- your name, if not the applicant
- full names of both parties, if for a marriage or civil union
- your email address
- your phone number.
