If you have a question about a Births, Deaths and Marriages (BDM) order or need advice, our kaimahi are here to help.

Information we need from you

To help us answer your query, have the following information ready to tell us over the phone or by email.

For a general enquiry

Provide:

  • your full name
  • your email address
  • your phone number
  • the BDM product or service you’re enquiring about.

For a current or past order

Provide details of the order including:

  • the type of product or service applied for
  • the applicant’s:
    • full name
    • date of birth
    • delivery address.

Example

A birth certificate for:

  • John Michael Smith
  • date of birth — 1 February 2012
  • delivery address — 1A Queen Street, Auckland 1010.

Also provide:

  • your name, if not the applicant
  • full names of both parties, if for a marriage or civil union
  • your email address
  • your phone number.
